What information do we collect about you?

We will collect information such as personal details, including name, address, next of kin, records of appointments, visits, telephone calls, your health records, treatment and medications, test results, X-rays, etc. and any other relevant information to enable us to deliver effective medical care.

We will contact you from time to time to invite you for reviews and updates on information such as your smoking status, test results or when we need to contact you about your care.

The information you provide us is processed by an IT provider approved by NHS England. Your health information is encrypted and transferred to be stored outside the surgery in a remote location.

We will provide information to other organisations for compliance with legal obligations.
We also provide information to organisations who commission NHS services such as the local CCG, NHS England.
When needed, your information will be shared with organisations providing your care: Hospitals, clinics, social services and Public Health England.
Your information is collected by relevant authorities for the purpose of preventative medicine, the provision of health or social care or treatment or the management of health or social care systems and services

We are committed to maintaining confidentiality and protecting the information we hold about you. We adhere to the General Data Protection Regulation (GDPR), the NHS Codes of Confidentiality and Security, as well as guidance issued by the Information Commissioner's Office (ICO).
